Actually, I read somewhere that in the mother's pouch, there is some kind of bacteria or fungus that create enough oxygen for the baby to breathe! But anyway, usually fabric is pretty breathable, but if you want, you can get an eyelet kit at any cloth or fabric place or even at Walmart, and make some air holes, if you are a little nervous about it. I make my own pouches, and I always put a few eyelet holes in there, depending on how thick the fabric is.
