My version is :

1 (4 oz) bottle Gerber Banana Juice w/yogurt (& added calcium)
1/2 cup honey
1 egg, scrambled or boiled w/shell
1/2 cup baby cereal (rice with mixed fruit)
2 teaspoons RepCal (I use Reptocal) must have vit. D3
1/4 cup wheat germ
1 (2 1/2 oz) jar of (stage 2) Heinz chicken baby food

Mix well in blender. Freeze in Tupperware bowl. Thaw 1 tablespoon per
glider each night.
I add (after scooping into bowls or cups) frozen mixed veggies and stir
I also feed fresh fruit every night. (mine don't like frozen fruits)
Insects are also part of their diet.
This will feed two sugar gliders for approximately 1 month.
