As stated by Maria and April, definitely change diet for good.
As for weening, the mother will take care of that herself.
Carrying her with you should pose no problem as long as this is what she is used to and doesn't object. Kim's SG Cher birthed her joeys in Kim's hand. If bond is strong enough she will be better off doing what she normally do.
I would be interested in where the father is, though.
